Before Your Esthetics Appointment

The following forms are required for the related esthetics services. Simply click the link to fill and sign online and your completed form will be sent straight to our administrative staff! There is also the option to print out the forms and bring them in completed at your appointment time.

• Clients may request a particular student for certain services, however, the school assigns esthetics services based on the skill level of the service and the academic requirements of the student.
• The school reserves the right to change appointments based on student’s attendance.
• A patch test may be required prior to performing a Tier 1 or Tier 2 chemical peel or other services as determined by the instructor.

*Children are not allowed in the spa; visitors may wait in the lobby.
